How to interpret it safely

Short answer: Axial-length percentile shows where a child’s eye length sits compared with peers, but it is not enough by itself. A high percentile, fast measured growth, young age, family history, and treatment response together tell the clinical story.

What percentile means

An axial-length percentile compares a child’s measured eye length with an age- and sex-matched reference population. A child near the 90th or 95th percentile has longer eyes than most peers, but risk interpretation still depends on refraction, age, and whether the eye is continuing to elongate quickly.

Percentile vs growth rate

High percentile, stable growthStructural risk may be elevated, but treatment response may be acceptable if follow-up growth is slow.
Average percentile, fast growthMay still need careful monitoring because trajectory can worsen before percentile looks alarming.
